ABOUT THE ROLE We are looking for exceptional Care Coordinators who are passionate health / aged care professionals looking to join our Caloundra, Caboolture & Gympie offices on a full-time basis. As Care Coordinator, you will be an integral point of contact for workers and clients to provide support for the broader community team.
As a Care Coordinator, you will be responsible for: Building strong and meaningful connections with clients and their family Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home Interacting and providing professional health and clinical care advice and direction to a team of home care workers Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction Ensuring all practices are consistent within policy and legislative frameworks Maintaining up-to-date client records and information To be successful you must hold: Formal tertiary qualification eg. Bachelor of Nursing or equivalent Current AHPRA registration as an Enrolled or Registered Nurse Demonstrated knowledge within the Aged Care sector National criminal history / police check Industry applicable licences (eg.
