Key responsibilities include but are not limited to: Ensuring client care is assessed, planned, implemented, and evaluated in accordance with BlueCare's Tailor-Made Service Model, as well as funding models, taking into consideration each client's physical, social, emotional, spiritual and clinical needs and encouraging client/client representative participation. Complete regular review of services ensuring effective schedule of client services and maximising package spends. Act as a key contact for clients through their Blue Care Journey – and be a day-to-day entry point for services and general enquiries, plan enhancements and changes to funding. Onboarding and offboarding clients, and management of client information in CRM, ensuring up-to-date details and case notes. Assist with orientation of new staff and ongoing professional development, and fostering and driving a high-performance culture and providing great quality of life aligned to the BlueCare values and mission. What makes a difference for us? You'll have extensive experience in nursing or allied health or human services and demonstrated understanding and sound knowledge of client-centred, community-based support. You will have extensive stakeholder management capabilities and be able to deliver consistently professional and innovative person-centred care.
Tertiary qualification in health/community service or other relevant discipline Able to satisfy the required probity checks by legislation or Blue Care policies Ability to liaise with clinical and care team to provide coordinated care as per the package Excellent written and verbal communication skills BlueCare is proudly part of the UnitingCare Family, we'd love you to be too!
