Aboriginal Health Worker - Connected Beginning - Aboriginal Identified Note: This is an Identified position for Aboriginal and Torres Strait Islander People. Our main services are in Wellington, Dubbo, Moree, Western Sydney, Penrith, Nepean, and Blue Mountains, and we provide outreach services to other towns and communities through our regional programs.
About The Position The Connected Beginnings Aboriginal Community Health Worker is responsible for facilitating healthcare screening and access to children referred into the Doonside and/or Mount Druitt Connected Beginnings Projects. This includes engaging families for health care from birth to five-year-old children to facilitate better outcomes by the time they start school with a focus on the following domains:
Physical health and wellbeing Social competence Emotional maturity Language and cognitive skills Communication skills and general knowledge This project delivers the health components of the Connected Beginnings Project that are led by education agencies Ngroo, Mount Druitt and Doonside. The position reports directly to Team Leader Connected Beginnings.
Key Duties Maintain positive partnerships with key stakeholders Engage Aboriginal Families and Children in their healthcare journey Perform health and developmental screening and referrals Written communication in files, letters and report writing Contribute to data collection for the Connected Beginnings Project Facilitate culturally appropriate healthcare for Aboriginal children living within Doonside and Mount Druitt regions Participate in service planning with services and agencies as directed by the Team Leader Participate in quality and accreditation activities as per the direction of the Team Leader Excellence in teamwork Experience in case management of clients Demonstrated time management and the ability to prioritize tasks Knowledge and understanding of the issues that impact Aboriginal people and Aboriginal communities Previous Experience in Aboriginal Health is highly Preferable Mandatory Compliance Requirements of the Position: A Certificate of Aboriginality Certification III Aboriginal Health Worker (willing to obtain) Previous Experience in Aboriginal Health is Highly Preferable Australian Drivers Licence Working With Children Check Proof of COVID vaccination National Criminal Record Check Your application will include the following questions:
