Centacare is looking for Triage Support Officers to provide adaptive support for our assessment teams. We have positions available at our multiple office locations anywhere between Gold Coast, Brisbane, Logan and Sunshine Coast so discuss with us what works best for you. Responsibilities: Perform initial needs assessments and determine eligibility for further assessment.Direct the assessment pathway by assigning clinical or non-clinical assessors.Determine the priority and urgency of care required for clients.Ensure accurate and timely triage processes.Role Requirements: Tertiary qualifications in a health-related discipline (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or other Health Professional).At least one year of experience in a clinical or triage role is preferred.Minimum 1 year aged care needs assessment experience within aged care settings and/or to aged persons (geriatric evaluation, rehabilitation, palliative care, community nursing) including people with dementia.National Police Check.Unrestricted Australian Drivers Licence.
