About the Role: The North Coast Region is seeking to employ six (6) Transitions Pathway Officers until the 31st of December, 2025, based in Bundaberg, Wide Bay, Gympie, Sunshine Coast, and the southern corridor of the North Coast Region.
As the Transitions Pathway Officer, you will possess or have the ability to rapidly acquire an understanding of departmental policies and procedures.
The role will require you to have experience in supporting young people, their families, and schools.
Additionally, an understanding of services available through the Department and their local community to assist students transitioning into their senior phase of learning or into a suitable post-school education, training, or employment pathway is highly desirable.
Travel is a requirement of the role, and access to your own reliable transport is essential.
Responsibilities include: Support schools to interpret, develop, refine, and enact departmental policies, practices, and procedures relating to students exiting school prior to the completion of Year 12 to ensure they remain in an eligible option.
Provide support to schools that will assist in the development and implementation of successful retention and transition strategies and programs that will improve the transition outcomes for 'at risk' students.
Provide appropriate support through the identification of student needs options and intervention to young people and their families to assist in the transition of young people into viable education, training, or employment.
Collate and analyse confidential information, identifying needs and barriers to formulate the basis for the development of a school to post-school transition plan for young people (post compulsory participation phase) who leave school before the completion of Year 12 (transition of students in the compulsory participation phase to be determined by region) and provide appropriate case management, advocacy, and support as required.
