What you will be doing: Ensure responsibility for allocated client/s and their home (if applicable) overseeing all practice, process, and operations Support the implementation of NDIS Quality Standards and Human Rights for all client/s, always demonstrating best practice Support Case Manager with full compliance of client profiles and assist with the implementation and review of relevant support plans (incl. ISP, PBSP, safety plans etc) Create and implement weekly client Activity Plans and meal plans as needed with input from the client Lead the team and build a positive team spirit, whilst ensuring a strong focus is placed on client goals and their outcomes Coordinate correspondence with both external parties and the team and ensure correct and up to date SCSS and government information is always available for your team Ensure all required documentation is completed by the team in a timely manner Contribute to stakeholder and guardian communication as required and directed by Case Manager Plan and lead Core Team Meetings and complete shadow shifts with all new team members Complete weekly internal audit checks What you will bring: Flexibility across a 7 day/night roster, which can include shift work, periodic weekend, public holidays, night shifts & overnight sleepovers. Experience in a similar senior position (desired but not essential) Flexibility to support across additional disability programs Ability to ensure attention to detail and time management with client documentation and shift reports Blue Card for Working with Children & NDIS Worker Screening (or Yellow Card) Driver's licence and own vehicle with comprehensive insurance NDIS E-Learning modules What we can offer you: Ongoing Learning & Development including CPI training Annual FREE Flu Vaccination Reimbursement to maintain First Aid & CPR CEO Scholarship Program Employee Assistance Program Corporate Health Care Partners Location Swap – locations across QLD Recognition – Employee of the Month, 5 & 10 year Service Awards Interested in this job?
