Position Title: Community Nursing and Home Care ServicesClassification: ON14 - ON40 Dependant on QualificationSalary per hour (exc. super): $45.76 - $49.96Hours per fortnight: 64 hoursEmployment Status: Ongoing Part Time Closing date: 20/11/2024
Community Nursing & Home Care Service (CN&HCS) provides holistic health care to people who are frail aged, younger people with a disability, and their carers to maintain independence. The service combines a range of clinicians which support clients with their individual needs.
CNS and HCS incorporates:
District Nursing ServiceRegional Continence ServiceRegional Wound Management ServiceRegional Dementia Consultancy ServiceHome Care WorkforceCN&HCS aims to improve independence, health, wellbeing, and quality of life and prevent premature admission to hospital or residential care for older people, younger people with disabilities, and their carers. CN&HCS nursing staff encourage clients to participate in their care and maintain their independence.
As part of the Management Team, the Team Leader Home Care Workforce provides leadership, direction, and support to the Home Care Workforce to facilitate and coordinate the delivery of high-quality, best practice service provision to people with complex needs and older people living in the community, in a timely and responsive manner.
The Team Leader will:
Utilise an innovative approach to develop and expand the scope of the Home Care Workforce in response to changing demand, lead and implement opportunities for continuous improvement and change within the teamLead, support, and provide direction to the Home Care Workforce in their daily roles to ensure best practice is followed and is compliant with required policies and proceduresEnsure the delivery of high-quality home-based services to consumers of Bendigo Health, including but not limited to: personal care, social support, and domestic assistance.Develop and foster collaborative relationships across the health service and community sector to provide high quality care provisionDevelop initiatives to ensure Bendigo Health is an employer of choice for relevant workersParticipate in and contribute to the Community Nursing and Support Services leadership team.Working with Bendigo Health Bendigo Health is a leader in regional health care, combining outstanding services with a strong education, training, and personal development culture.

Your New Division The Community and Public Health Services Division has a primary focus on improving the health outcomes of the communities in our region. With a range of local and regional programs supporting place-based health promotion, prevention, care, and support, they work collaboratively with other Divisions and regional partnerships and networks to support quality outcomes. The Division has a strong leadership role in the Loddon Mallee Region and is committed to ensuring processes are in place to deliver on our Division.
