About us Grand Pacific Health is a not-for-profit, primary health care organisation.
We're here to provide our local communities in NSW and the ACT with equitable, inclusive and integrated health care including physical health and mental health care services.

About the position The Support Coordinator for The Way Back Service plays a crucial role in providing non-clinical, practical support to individuals who have recently attempted suicide or experienced a suicidal crisis.
This service offers up to three months of personalised, psychosocial support during the critical period following a suicide attempt or suicidal crisis, aiming to foster recovery and reduce future risk.
The Support Coordinator provides direct support to consumers, and is responsible for identifying and assessing potential risks to consumer safety, including conducting regular risk assessments and addressing warning signs of suicidal ideation or behaviour.
In addition they work closely with consumer, adopting a strengths-based approach to help them develop tailored support plans, connect with community services, and monitor their progress.
In addition to these responsibilities, they implement comprehensive safety planning and risk mitigation processes, ensuring consumer safety and continuity of care.
The role also involves assisting consumers in navigating a network of regional services, helping them access community resources, mental health services, and social supports essential for their recovery.
By fostering hope and resilience, the Support Coordinator empowers consumers to take meaningful steps in their recovery journey in the setting most comfortable for themwhether at home, in the community, or on-site.
This is a part-time fixed term opportunity until 30 June 2025 (with possibility for extension based on funding); hours and days are negotiable.
We are looking for someone with:  Relevant qualifications (minimum Cert IV or other relevant qualification in mental health or human services), and/or experience in suicide prevention or mental health work.
Experience in understanding personal, family, and social factors related to suicidal ideation and attempts, including awareness of groups at higher risk of suicide Demonstrated case management skills and ability to work effectively with consumers, family members and service providers to identify solutions.
