As a Support Coordinator, you will play a vital role in empowering and supporting individuals with disabilities to live independent and fulfilling lives.

What you'll be doing Coordinating and overseeing the delivery of individualized support services to clients, ensuring their needs are metAssisting clients in developing and implementing their goals and support plansLiaising with a range of service providers and stakeholders to ensure seamless service deliveryMonitoring and reviewing client progress, and making adjustments to support plans as neededAdvocating for clients and their families to ensure they have access to the appropriate services and resourcesMaintaining accurate and up-to-date client records and documentationParticipating in team meetings and contributing to the continuous improvement of service deliveryWhat we're looking for Diploma in Disability or Community Services related (desired but not necessary)Minimum 1 year experience in a support coordination or case management role, preferably in the aged care or disability sectorsExcellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with clients, families, and stakeholdersStrong organizational and problem-solving skills, with the ability to prioritize and manage multiple tasksDemonstrated understanding of the NDIS and aged care systems, and the ability to navigate and coordinate services within these frameworksEmpathetic, person-centered approach with a genuine commitment to making a positive difference in the lives of clientsCurrent driver's licence and access to a reliable vehicleAbout us At NDiCare our main focus is assisting individuals to reach their full potential.
