Responsibilities:
As a Support Coordinator at Approach Care, you will play a pivotal role in empowering our clients by:
Participant Support: Assisting participants in understanding and implementing their NDIS plans.Developing individualized support plans tailored to participant goals and aspirations.Providing guidance and advice on available services and supports.Coordination and Management: Coordinating access to various services, including therapy, health care, education, and community participation.Managing participant budgets effectively to maximize plan outcomes.Building and maintaining relationships with service providers and stakeholders.Problem Solving and Advocacy: Supporting participants in resolving challenges or barriers in accessing services.Acting as an advocate to ensure participant rights and needs are prioritized.Promoting self-advocacy by empowering participants and their families.Compliance and Reporting: Ensuring adherence to NDIS guidelines and standards in all activities.Maintaining accurate and comprehensive participant records and reports.Staying updated on NDIS policy changes and their implications for participants.Qualifications:
To thrive in this role, you should have:
