NDIS Support Co-ordination assists participants to find the right services and supports to meet their individual needs and assist in achieving their goals.
What you will be doing Provide coordination of supports to NDIS participants and assist them in achieving the goals set out in their NDIS plansAssist participants to find services, both funded and mainstream to participate more fully in the communityAssist with building informal and formal supports, resolve issuesAssist participants to strengthen their own abilities to coordinate and implement supportsWhat's in it for you? )Access to contemporary in-house supervision as well as a nationally recognised Employee Assistance ProgramWho we are looking for: You'll have a tertiary qualification in Social Work, Psychology, Human Services OR a Certificate IV/Diploma level qualification and significant experienceHave demonstrated skills and experience in working with children, young people and adults, who meet the eligibility requirements of NDIAIntermediate ability to use computer IT systems including MS Word and ExcelValid working rights in AustraliaHave a Queensland Provisional Green or above Driver LicenseHave a NDIS Yellow Card and Working with Children Check (Blue Card) and any other legislatively required personal history screening as required.If this sounds like You?
