You will provide support and advise participants living with a disability to receive the services and support required to maintain their wellbeing. This position is a full-time position based in Ararat.
Your responsibilities will include:
Providing advice and coordination services to participants to allow choice and self-management to ensure the required services are received Plan and implement services and supports that empower participants to participate in activities. Promote best practice 'person centred care' Provide participant services to address their wellbeing Maintain networks and knowledge of services and liaise with service providers. About you
You will be an experienced passionate Support Coordinator with outstanding communication and client service skills allowing you to build strong relationships with a diverse range of people. You will have excellent time management and organisation skills with the ability to manage competing demands to meet client needs.
Requirements of the role:
Relevant industry experience and tertiary qualifications in Disability or Community Services Knowledge of and ability to map out participants NDIS Budget for the provision of services and supports Knowledge of NDIS, Disability Act and other relevant legislation A Victorian driver's licence, NDIS Check and a Working With Children's check If you have the passion and skills required for this rewarding role, please Apply Now.
