Role Responsibilities: Coordinate both formal and informal supports Manage clients with complex needs and handle crisis situations Collaborate with a Multidisciplinary Panel Engage with Mental Health, Justice System, Housing, and Health sectors Undertake clerical administration and report writing Network and communicate with key stakeholders Work independently, either from home or on-site.
Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.

Qualifications & Experience: Relevant qualification in the Human Service Sector or a minimum of 3 years' experience in a similar disability sector role Preferred: Degree in Social Work, Psychology, Occupational Therapy, Counselling, Human Services, or Disability Studies Experience in Forensic Disability or Forensic Mental Health is a plus Demonstrated knowledge of the NDIS and experience working with its participants Valid Driver's License with a suitable, insured vehicle A current Working with Vulnerable People registration Recent police check (or willingness to obtain one).
