About the role:
This position is based with the Acute & Continuing Care Team (ACCT). The team is responsible for providing acute response, assessment, planning, and case management for adults living in the community who are experiencing an acute episode and may require continuing care and case management. The Acute & Continuing Care team provides mental health support for adults (18-64) who are experiencing a mental illness.
The recovery-oriented approach is person-centred, trauma-informed, and involves partnering with general health providers and agencies that can offer clients community living supports such as housing, disability services, vocational, social, and educational opportunities.
