Child Welfare, Youth & Family Services (Community Services & Development) Role OverviewAs a Case Manager at Partner in Care, you will support NDIS participants by ensuring they have access to the services and supports they need. This includes managing a caseload of clients with complex physical and health needs, working to develop individualized care plans, and coordinating services across a wide range of providers. You will play a pivotal role in helping participants navigate the NDIS system, achieve their goals, and improve their quality of life.
Key Responsibilities Care Coordination: Manage and coordinate a caseload of clients, overseeing the delivery of services in line with their NDIS plans and ensuring services meet their needs.Client Advocacy: Advocate for clients' rights and ensure they are accessing all services and supports to which they are entitled under the NDIS.Support Plan Development: Collaborate with participants, families, and other professionals to create and review personalized care plans, anticipating changes in physical and health needs.Monitoring and Evaluation: Track participant progress through regular reviews and adjust care plans as necessary to accommodate changing circumstances or needs.Crisis Intervention: Provide immediate support in response to client crises, connecting them with emergency or specialized services when required.Stakeholder Collaboration: Work with a multidisciplinary team and external providers to deliver holistic care, including healthcare professionals, therapists, and community support workers.Service Navigation: Help participants navigate intersecting service systems such as Health, Housing, Justice, and other community services.Budget Management: Assist participants in managing their NDIS budgets, ensuring the most effective use of their funds for services and supports.Report Writing: Complete implementation, progress, and annual reports for NDIS clients, ensuring all documentation is accurate and up to date.Required Qualifications: Degree in Social Work or psychology (Bachelors/ Masters)Prior experience in Support Coordination, Case Management, or similar roles in the disability sector.Knowledge of NDIS processes, particularly in working with clients with complex health needs.Strong interpersonal and communication skills, with the ability to advocate effectively for clients.Ability to work both autonomously and collaboratively within a multidisciplinary team.Preferred Skills: Experience working with clients with physical disabilities or chronic health conditions.Familiarity with psychosocial and mental health issues, particularly in the context of NDIS support.Ability to manage multiple tasks, prioritize, and meet deadlines in a fast-paced environment.Workplace Culture:At Partner in Care, we pride ourselves on creating a supportive, inclusive, and dynamic work environment. Please note: Participation in clinical on-call arrangements (including the organisation's Emergency Clinical Oncall Roster which requires a 30-minute response time), shift work (including early, late and night, as well as weekends and public holidays) is required.
This is a risk assessed role: Possession of a current BlueCard and NDIS worker Screening Card are a must for this position.
