They will coordinate CHSP services, monitor and respond to changing client needs and promote health, wellbeing, and continuity of care, so our clients can age in place.What will this look like on a day to day basis?You will be:Conducting client assessments and reviews with clients and their families/representatives in accordance with the CHSP guidelinesDeveloping and monitoring client-focused, comprehensive care plans, and assisting with implementationMaintaining high-quality assessment and care planning documentationFostering a collaborative and respectful relationship with each client to fully understand the clients' needs and ensure our services are reflective of person-centred practice and the ACQSProviding comprehensive case management, monitoring physical and emotional wellbeing of our clients and identifying longer-term plans enabling them to stay connected to services after discharge from the programWorking collaboratively with other team members, community organisations, allied health providers, and service agencies to ensure the best outcome for the client and their careManaging and recording feedback and complaints regarding service delivery in line with internal policies and ACQS requirements, undertaking remedy actions to address issuesProviding regular reporting of client and service status, and monitor output targets and deliverables under our CHSP funded servicesOther things you might like to know:6-month contract position with potential to extendSome travel required (kilometre reimbursement offered)Paid compliance checks including NDIS Worker ScreeningWhat skills and experience do you need to be a Care Advisor?This position is best suited to someone who:Cert IV in Community Services, Human Services, Health Sector Management, Aged Care and/or Clinical background and minimum of 3 years' experience in the industryDemonstrated understanding of the Aged Care reform, Aged Care Standards, the sector and people who are aging in the communityExperienced in case management preferably within the home and community care sectorExperience in service development, and driving continuous quality improvement and practice and service excellence aligning to Aged Care Quality standardsIs relatable and engaging with highly developed interpersonal and communication skills and emotional intelligence with the proven ability to foster relationships and connect with internal and external stakeholders.How to applyBe creative!
