About the Role:

The Northern Adelaide Local Health Network is seeking a compassionate Social Worker to join the dedicated Northern Adelaide Palliative Service team.
This is a rewarding opportunity to contribute to the lives of individuals and families facing end-of-life challenges.
In this role, you will work closely with a multidisciplinary team, providing exceptional psychosocial support and advocacy for patients and their loved ones during their palliative care journey.
Your expertise will be invaluable in assessing complex social and emotional needs, facilitating difficult conversations, and ensuring seamless care transitions.
As a key member of our team, you will play a crucial role in: Conducting comprehensive psychosocial assessments and developing tailored care plans Providing counselling and emotional support to patients and their families Facilitating family meetings and ensuring effective communication Advocating for patient rights and promoting informed decision-making Collaborating with healthcare professionals across various disciplines

Minimum Qualifications:

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
