Social Worker Grade 2- Rural Allied Health Employment Type: Part Time Fixed-Term position until end of June 2025(maternity leave replacement) Location:Shepparton About the Hospital:

About the role
Team with a well renowned supportive workplace culture with an opportunity to work alongside experienced Allied Health Professionals.In this role you will work autonomously, undertake a varied and diverse workload and provide interventions in predominately home-based settings, making a true difference to our client's independence and well-being. There is no nursing home work.Our multidisciplinary team includes Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work and Speech Pathology services, supported by Allied Health Assistants.A comprehensive orientation program is available.

The Social Worker will provide person centred care with an opportunity to:
Travel across the Goulburn Valley region working in home-based settings.Work with clients, carers and families to provide support, counselling, information, advocacy advice and referral with a focus on informed decision making and empowerments solutions to promote independence home and in the community.Develop goals and care plans to assist clients to remain at home and active in their local community.Advise on strategies and areas of assistance to support clients to remain living independently at home and within their local communities.Work with a diverse range of clients and health conditionsParticipate in student training programs.
