super): $44.77 - $51.46 Hours per fortnight: 64 Employment Status: Ongoing Part Time Closing date: 22/12/2024 The purpose of this position is to provide best practice client centred clinical care to clients within the designated caseload in TCP/GEM@Home/RITH, providing emotional support to care recipients and their families/carers, whilst contributing to care planning and discharge planning.
Social Workers will review clients and their carers either in their own homes and/or in a bed-based setting.
The TCP/GEM@Home/RITH Social Worker will utilise skills and experience for assessment, intervention and education appropriate to client and carers need.
