Social Worker | Blue Care | Telehealth & Community Care Employment Term: Full-Time Permanent contract. A hybrid role with Telehealth and Face-to-Face Community Care options. You will be providing critical mental health and wellness support to BlueCare's in-home Community aged care customers. Your role will deliver high-quality, clinical support to clients, working in partnership with a collaborative, multi-disciplinary team to deliver an exceptional standard of person-centred outcomes.
You will provide our clients with Social Worker services with specialist advice and clinical services to improve their quality of life and well-being!
This includes developing and implementing treatment & intervention programs, case management, clinical counselling, debriefing following critical events and education & support for clients and carers.

Tertiary Qualifications in Social Work Registration with relevant professional body - Australian Association of Social Workers Previous experience within an Aged Care or Acute setting (desirable) Excellent interpersonal and written communication skills Experience with technology incl. smartphones, tablets, laptops etc Reliable internet connection Current Australian drivers licence and roadworthy registered vehicle What makes a difference for you?
