We have an exciting opportunity for a Service Performance Improvement Coordinator to join our future-focused and rewarding organisation.
This role reports to the Executive Director, Strategic Commissioning and is a key member of the Commissioning Team. Darling Downs and West Moreton PHN is a not-for-profit organisation that works at a regional level to achieve improved whole of system health care.
Our key objectives are to: Increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes.Improve coordination of care to ensure patients receive the right care, in the right place, at the right time.About the role: As the Service Performance Improvement Coordinator, you will be tasked with a wide variety of responsibilities that assist with the continual maturity of the organisation including but not limited to:
Operationalising the Performance and Outcomes Framework in the context of commissioned programs to ensure that effective monitoring, evaluation and learning are embedded across all services and initiatives.Leading the ongoing development of data maturity and continuous quality improvement initiatives for external and internal stakeholders.Enabling the implementation and ongoing development of the data management system.Key purpose of the role: Implement the DDWMPHN Performance and Outcomes Framework.Drive the development of a core suite of KPIs and measures.Support the benchmarking and baselining of KPI targets.Apply expert knowledge in the collection, validation, storage and analysis of program and other data.Lead initiatives to improve data maturity across program teams and service providers.Lead the implementation and coordination of the data governance structure.Lead and/or participate in data system development projects.Oversight of the data management system vendor contract and performance monitoring.Develop and maintain effective relationships with stakeholders.Support DDWMPHN to ensure client privacy and confidentiality, and data security are maintained.Required Attributes: Highly developed operational and strategic understanding of key data sets across primary health contexts.Highly developed IT skills including Microsoft Excel and data analysis programs.Experience in leading data governance, quality improvement, and project implementation processes.Ability to analyse, interpret and transform complex information.Understanding of national and statewide directions in primary health services reform.Highly developed interpersonal and communication skills.Demonstrated experience working collaboratively with teams.Exceptional organisational skills with proven ability to deliver outcomes on time.Tertiary qualification and/or relevant work experience in public or population health.Highly developed understanding of data maturity and governance.Highly desirable: knowledge of trends in primary health care related to health data.Benefits of working with us: Great remuneration including salary packaging.Challenging and rewarding role.Friendly and supportive team environment.Generous leave entitlements including a monthly RDO and 5 weeks annual leave.All enquiries should be directed to Kate McLoughlin via email ******.
