Service Delivery Coordinator

Summary: We are seeking a highly organised and detail-oriented Service Delivery Coordinator to join our team. The Service Delivery Coordinator will be responsible for ensuring the timely and efficient delivery of services to our clients, you will play a critical role in planning, coordinating and managing services for individuals with disabilities ensuring they receive the necessary support to lead fulfilling lives.
Responsibilities: Develop, implement and monitor individualised support plans Advocate for client's needs and ensure they receive appropriate supports and services Monitor service delivery performance and identify areas for improvement Develop and maintain strong relationships with clients and stakeholder groups, conduct regular assessments and reviews of clients progress. Provide regular updates to the relevant stakeholders on the status of the clients service requests Ensure that all service delivery processes are documented and adhered to Identify and escalate any issues or concerns to management as needed Qualifications: Minimum 2 years of experience in service delivery coordination or a related field (desirable) Strong knowledge of disability services Excellent communication, interpersonal, organisational and problem-solving skills Ability to work independently and as part of a team NDIS Yellow Card and current Drivers Licence (mandatory) First Aid and CPR - or ability to obtain Proficiency in Microsoft Office and other relevant software applications
