Service Coordinator - Toodyay position available $88,963 per annum + super Flexible working conditions to support work-life balance 12 month fixed term position with a view to permanency About the role This position is responsible for developing Avivo's Community Teams in Toodyay area to support customers to live a good life within their community.
The Service Coordinator works in partnership with customers to design and develop supports to enable them to: Lead fulfilling lives in a way that makes sense to them Achieve their goals and outcomes Maintain their independence Be a contributor and valued member of their community Maximise the use of their funding The Service Coordinator is responsible for ensuring quality support and services are provided to customers within the area and sourcing resources needed.
They work closely with teams, supporting them to provide excellent services to customers and ensuring they have the knowledge, skills, values, and resources required.
This position works in partnership with colleagues within the region and across Avivo to develop knowledge and skills in working with particular funders and target groups, i.e., disability, aged care and/or mental health.
They contribute to the communities of practice that focus on the above mentioned.
Duties include Leadership, recruitment, and development of the team of field-based staff Planning and providing services against budget within their geographical area Supporting the team with knowledge and skill Working within a person-centred framework to ensure support is provided in the most efficient way possible by working closely with the Mentors, Team Coach, Service Coordinator Team, Resource Team, Team Facilitators and Support Workers.
Selection criteria Previous experience in a similar role of 2 years or more (essential) Certificate IV in Disability, Aged Care, Mental Health (or working towards) Previous experience in supporting and developing employees and teams Demonstrates ability to work as a supportive leader, with ability to coach and resource others' development Knowledge and understanding of disability, aged care and/or mental health sectors and demonstrates contemporary professional knowledge, values, and practice Commitment and skills in providing excellent support to customers Understanding and commitment to working with communities to welcome and include all people as valued citizens Excellent interpersonal and communication skills, numeracy, and literacy Strong IT skills including ability to use portals, systems, and document relevant information About you You're a passionate and supportive leader with a goal to achieve a real difference in your local community, being committed to organisational transformation and ready to take on new challenges.
