About the Opportunity This opportunity will suit a natural leader or experienced support worker/carer with a strong social conscience and genuine commitment to: Improving the lives of people with a disability Coaching support workers to develop and implement best practice, person-centred active support Creating/strengthening roles for people with disability in their communities Expanding HomePlace's network of like-minded organisations, so that together we can expand the experiences and community contributions of the people HomePlace supports The primary function of this position is to provide direct support to individuals who have contracted HomePlace as their provider.
Additional responsibilities are: Facilitate connections and relationships that assist in creating social and economic participation for the person being supported!
Orientation of new team members Mentor and Support team members Oversee the day-to-day management of the supports in the person's home Facilitate team meetings and effective communication Provide person centred active support with participants over a 7-day weekly schedule which will include early mornings, evenings, weekends, sleepovers Participants may seek your support or mentorship in a range of activities that could include: Sports Performing arts including singing, dance Arts and crafts Gardening Volunteering Developing and working in their own micro-enterprise/business Travel experiences Cooking Health and fitness The role additionally has a portfolio in continuous improvement and/or project management coaching and support for projects across HomePlace.

To be successful in this role, you will have/be: A minimum of two (2) years of experience in working with people with intellectual or other disability and their families Extensive experience in supporting individuals with complex health care needs and delivering high-intensity support in a person-centred manner Confidence in assisting with behavioural challenges Proven experience in implementing Positive Behaviour Support (PBS) strategies to enhance participant well-being and outcomes Certificate IV specialising in Disability Studies and/or Human Services and/or substantial experience in this or a similar role Experience supporting or coaching support staff in community settings Continuous improvement methodology Experience with project management Excellent interpersonal and communication skills, written and verbal Good organisational and time management skills with the ability to work using own initiative Ability to prioritise tasks and adopt a flexible approach to working methods and time Ability to enthuse others and be a good team player Proficiency in the use of IT, in particular MS Office Current NDIS Clearance Senior First Aid / Basic Emergency Life Support, and CPR certificate Hold a current drivers licence, have access to a vehicle, and be willing to drive to undertake HomePlace business COVID Vaccinated x 3 It would also be great if you had: Experience in providing support to individuals with mental health needs Experience working for a disability service in a leadership role Experience with MYP About Our Client HomePlace supports people who live independently in their own homes and to be part of their community and enables individuals to be safe and supported to make continuous improvements in their quality of life.
