Senior Pharmacist - Community and Oral HealthInnovation in health care will be critical in improving our ability to meet individual and population health care needs. Models of care will need to be designed to encapsulate transitions of care and patient centred outcomes. Permanent, fixed term temporary full time and part time opportunity to deliver high level Clinical Pharmacy services, to ensure medication safety across the continuum of care, that are sustainable and deliverable across all COH community services or facilities (including Satellite Hospitals – Kallangur and Bribie Island) on a rotational basis and that align with organisational goals.
About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Provide and be individually accountable for high level Clinical Pharmacy services showing well developed clinical knowledge and skills in the management of a caseload.Provide verbal / written medicines information and education to all stakeholders which includes patients / their carer's, medical, nursing and allied health staff both within and external to COH utilising the best communication channel (telephone, telehealth, face to face, electronic) to ensure person centred care is the focus and transitions of care are seamless.Provide clinical practice supervision to undergraduate and postgraduate students, interns and HP3 level pharmacists, including monitoring and maintenance of professional clinical standards.Health Equity It is expected that all Metro North Health staff, including the incumbent of this role as a valuable member of the Metro North workforce, contribute to the health equity agenda and meet the intent of supporting the defined six actions that specifically meet the needs of Aboriginal and Torres Strait Islander people within the National Safety and Quality Health Service Standards (NSQHS), by actively supporting the elimination of racial discrimination and institutional racism; supporting increased access to health care; influencing the social, cultural and economic determinants of health; supporting the delivery of sustainable, culturally safe and responsive health services; and recognise the importance of working with Aboriginal and Torres Strait Islander peoples, communities and organisations to design, deliver, monitor and review the health and support services we provide.
