The single most important goal of AMAH is to create an environment that facilitates clinical recovery, supports individual recovery efforts and strives to minimise service system barriers to the recovery process.
MOBILE AGED PSYCHIATRY SERVICE (MAPS) MAPS provides care to patients living at home, including within residential aged care facilities. MAPS is an interdisciplinary team which undertakes assessment treatment and case management of older persons over the age of 65 years with mental health problems living in the Inner South East region of Melbourne.
About the role As one of the Senior Case Managers, you will be responsible for intake/triage, case management and/or consultation & liaison. You will have well-developed organisational skills and will provide a high standard of case management to clients and expert liaison with other stakeholders in keeping with relevant legislation, policies and procedures. You will exhibit a high standard of clinical care delivery and be a role model to other junior team members and students.
