Registrar - Rehabilitation - Mcu @ Home

This is a full-time role split equally between:
0.5 FTE allocated to the MCU@Home program0.5 FTE focused on referral/consult handlingMCU@Home program responsibilitiesAssess patients for suitability for the program and consult with the specialist.Complete admission and discharge paperwork.Review patients at home and discuss their progress with the consultant.Arrange and follow up on investigations, medication reviews, scripts, etc.Monitor patient progress and collaborate with the team and consultant.Organise telehealth appointments and other required appointments.Lead and attend weekly case conferences and family meetings.Participate in the after-hours and weekend on-call roster.Review rehabilitation referrals promptly and efficiently, discuss with the consultant, and arrange transfers for suitable patients to the rehabilitation ward.Continue reviewing patients not ready for transfer and update the referring team on identified issues.Decline unsuitable referrals after consultation with the rehabilitation consultant, and inform the referring team of the reasons. #J-18808-Ljbffr
