The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.
Your New Role
As you step into this exciting onsite full-time opportunity reporting to the Nursing Regional Manager, you become an integral part of our flourishing organisation in a newly created position dedicated to improving the health outcomes of our valued customers.
This will be achieved through providing clinical management and coordination for our clients and partnering residential aged care facilities. The role is for a motivated and passionate individual that can work well in an autonomous role and appreciate the freedom and flexibility in this role that will be based from participating Aged Care facilities. Regular travel to partnering homes to facilitate clinical review will be required. The position is advertised for Hobart, but travel to other areas may be required.
The Registered or Enrolled Nurse will work as part of a multi-disciplinary team to ensure efficient and high quality service delivery coordinating the clinical team with the support of the Nursing Regional Manager and administration team. The successful applicant will also be required to support the model by conducting business development to grow the resident base. Proficiency in computer usage is a must, and while prior telehealth experience is preferred, it is not mandatory.

Essential Criteria
Bachelor's or Diploma of Nursing.
Experience in problem-solving and professional collaboration.
IT skills and proficiency in documenting customer needs.
AHPRA Nursing Registration.
Aged Care experience, leadership preferred.
Current driver's license and reliable vehicle.
