Assist in planning, assessing and evaluating our clients' needs with the help of our multidisciplinary team, to promote positive health outcomes. The Registered Nurse provides clinical support to our clients and collaborates with our support workers to deliver exceptional care to our clients in the community.
Key Responsibilities Work with clients, carers, and families to identify clinical goalsDevelop and maintain clinical care plansWork within current Calvary policy and procedures and WHS guidelinesCoordinate and organise the provision of nursing care including clinical comprehensive assessmentsWork with HCP case managers and liaise with VHC program/DVA community nursing and other servicesAbout You AHPRA RegistrationRecent nursing/clinical experience in the last 5 years or evidence of a refresher courseEffective communication, interpersonal and organisational skillsInitiative and the desire to continually improve client careCapability to prioritise tasks and achieve goals within set timeframesCandidates with an understanding of the community nursing sector - Home Care Packages, Department of Veteran Affairs and Commonwealth Home Support Program will be considered favourablyA desire to join a team committed to the care of others in a mission-based organisationWhy work for Calvary?
