As a Registered Nurse at Achieve Australia, your primary goal is to provide person-centered quality clinical services to people with a disability. You will monitor and review health care plans, ensuring health needs are met through regular monitoring, health assessments, and tailored healthcare plans. Reporting to a Service Manager, you will be responsible for healthcare planning across our services in Ryde and Hornsby.
Role Responsibilities: As a Registered Nurse, you will deliver quality clinical services to Achieve Australia clients including:
Developing, monitoring and reviewing comprehensive health care plans and other clinical care plans for people with a disability who have a range of functional abilities and complex needs.Provide nursing care to clients with urinary catheter, PEG and bowel care needs, including providing coaching and oversight to support staff in these and other care areas.Providing clinical practice leadership to disability support workers and advice to service managers across a number of assigned locations and assisting in the coordination of clinical supports.Building work relationships with local community health partnerships, families and carers and with other stakeholders both within and external to Achieve Australia.To be successful in this role, you will need: Current AHPRA Registration.Excellent communication skills, both written and verbal.Sound clinical knowledge and decision-making skills.Demonstrated ability to work autonomously and within a team.Computer skills including data entry, email and calendar management.Understanding of human rights and challenges facing individuals with disabilities.Experience in working with and supporting adults and/or children with complex disability is desirable.Ability to transport clients in Achieve Australia vehicles (automatic gearbox) across the region. Candidates without a current NSW driver's licence are subject to a risk assessment.Ability to travel across locations in Greater Sydney.Current First Aid Certificate.Experience in working with Indigenous Australians would be highly regarded.Achieve Australia has been supporting people with disability for 70 years.
