About the Role:
We are seeking a passionate and dedicated Registered Nurse to join our team in a hybrid role based in Chermside. This is a full-time opportunity that combines both office and community nursing duties, offering a unique blend of responsibilities. You will have the chance to make a meaningful difference in the lives of individuals within the local community while contributing to a supportive and collaborative team environment.
Key Responsibilities: Provide professional nursing care in the community, traveling to patients' homes as required.Conduct clinical assessments, including wound care, medication management, catheter care, stoma care, and enteral feeding.Administer complex care services such as palliative care, bowel care, and oxygen management.Collaborate with GPs, allied health professionals, and discharge planners to ensure coordinated and holistic care for clients.Review and update client health records, including care plans, risk assessments, and incident reports.Offer clinical training and mentorship to community support staff, including home care workers.Ensure compliance with internal policies and legislative requirements, particularly in preparation for audits.Provide support and advice to patients, families, and healthcare teams, ensuring the highest standard of care is maintained.Assist in managing clinical risk and resolving issues efficiently based on incident report reviews and team feedback.About You: A compassionate, reliable, and dedicated individual with a genuine desire to support others.Strong communication and interpersonal skills, with the ability to build positive relationships with clients, families, and colleagues.Excellent organizational skills and the ability to work independently.Comfortable using technology for documenting patient care and managing schedules.Experience in community nursing or aged care is desirable but not essential.Proactive in continuing professional development and best practice initiatives.What You'll Need: Current Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA).Evidence of continuing professional development.NDIS Worker Screening Clearance / Criminal Record Check.Current CPR / First Aid certification.Valid Australian Driver's Licence.Reliable, registered, and insured vehicle (comprehensive insurance preferred).Previous experience in community care, health, or aged care sectors is preferred.Proof of vaccination (where applicable), such as a COVID-19 vaccination certificate.Why Join Us?
