Australian Unity's Aboriginal Home Health platform is excited and proud to be partnering with Ramsay Connect to deliver the Diabetes Using Our Strengths Service (DUOSS) pilot research program.
Your role As our Community Registered Nurse, you will lead the clinical delivery of the Diabetes Using Our Strengths Service (DUOSS) whilst also working in a collaborative multi-disciplinary team including working closely with our Diabetes Educator.
The role will work in a digitally enabled model of care and be responsible for the delivery of quality, clinical care for patients in managing their type two diabetes in a holistic and culturally appropriate way.
This is a full-time 12-month fixed term role located in and around Tweed Heads with some travel across Bundjalung Country required.
The project is funded by MRFF funding from the Australian Government's Targeted Translation Research Accelerator program, delivered by MTPConnect.

Face to face home visits and remote patient monitoringEvidence based chronic disease managementCare CoordinationClinical escalation of any concerning results, signs, or symptomsHealth coachingCommunity yarn up sessionsYour expertise and qualifications will include: A demonstrated ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people and to facilitate culturally appropriate group yarning sessionsRegistered Nurse with unrestricted AHPRA registration and community nursing experienceClinical skills in chronic disease management and delivering care within a multi-disciplinary frameworkExperience in working in virtual careExperience in providing diabetes managementLocated in or around Tweed HeadsAccess to a motor vehicle for work related travelTo perform this role, it is essential that the person is an Aboriginal and/or Torres Strait Islander person.
