The Role: There are Part Time and Full Time positions within the Complex Care Hub. This role works across Complex Care Hub and Family Healthcare Support, providing clinical care to families, and education and training to in-home support workers in the school and home. This is a state-wide program; therefore, regional travel will be required with adequate notice and planned accordingly.
Classification for this position: YU15 ($102,278 base salary per annum, plus superannuation)
You will be responsible for: Providing family-centred care through a holistic approach using systematic assessment of the child's medical needs in conjunction with their psychosocial, cultural, and spiritual needs.Planning and implementing nursing care for patients in the Post-acute Care service, Homecare Program, School Care Program, and Family Healthcare Support.Facilitating the training and assessment of support workers for in-home care for eligible patients.What you'll achieve: Comprehensive and systematic nursing assessment to improve the quality of nursing care.Using a range of assessment techniques to collect relevant and accurate data.Ensuring appropriate information is developed and maintained to support the delivery of community care.Providing clinical advice regarding non-urgent nursing or medical queries.Your skills and experience: Excellent interpersonal and communication skills.Demonstrated ability to work within a multidisciplinary team and autonomously as necessary.Excellent time management and organisational skills.Proven ability to work independently and demonstrated initiative in performing duties.Ability to work well under pressure and be flexible to changing priorities and environment.Minimum of 3 years pediatric nursing experience.If this sounds like you, click here to view the position description.
About Complex Care Hub: The Royal Children's Complex Care Hub is an interdisciplinary service that has been established to lead and coordinate the healthcare experiences for complex patients and their families. Patients with complex needs frequently require multiple teams, advanced technology, and medical equipment to support their care needs, both within the hospital and at home. There are multiple programs under the Complex Care Hub including Post Acute Care, Home Care, and School Care programs.
Family Healthcare Support works alongside Complex Care Hub to provide training and support workers to tier 1 Complex Care patients across the state.
