We have a growing clientele and are seeking a Registered Nurse to join our Nurse Care Coordinator team.This position offers flexible working hours to accommodate family needs (e.g., 9am-3pm, five days a week).Company Background: Right at Home Southwest Victoria is a local home care provider that started in 2022 and is quickly gaining a respected reputation. We provide a wide range of care services designed to help our clients stay in their homes, including companion, personal, nursing, respite, transportation, aged, disability, transitional, 24-hour, and palliative care.Purpose of Position: This customer-focused role involves consulting with families, preparing in-home care assessments, and drafting care plans to best meet client requirements. This position is responsible for care coordination duties and will help develop relationships with clients and their families, hospitals, clinics, and care facilities to deliver appropriate care services.Duties include:Dealing with a range of seniors and frail elderly customers and their families, handling home care, information requests, feedback, general inquiries, and complaints.Managing all client contact in an empathetic, friendly, and patient manner to build rapport, trust, and confidence in Right at Home.Providing client and family education on available services and the benefits of the care plan.Preparing in-home care assessments and drafting care plans to meet client requirements.Managing government-funded cases as well as private pay clients.Training and managing the performance of the care team.Participating in community activities or committees as requested.Ensuring the highest quality service is consistently delivered.Assisting management with business development opportunities across a range of referral sources.Operating and maintaining care planning and scheduling technology.Ensuring all documentation is accurately completed in Right at Home databases and software programs in accordance with organizational policies and legislative frameworks.QualificationsNational police check.Current First Aid and CPR certification.Reliable source of transportation.Current AHPRA registration & Professional Indemnity Insurance.Working with Children Check Certificate (preferred).
