Employed within the General Practice, the Aged Care Coordinator will typically be a nurse with specialised training in aged care. This role is the primary link between the GP, RACH staff, and the older adult and their families, coordinating across the General Practice, the RACH, and other allied health services.
Mandatory RequirementsBachelor of NursingRegistration with AHPRACurrent Police CheckA passion for Aged Care and minimum of 5 years Aged Care Experience, preferably at senior levelHigh level communication skillsGraduate Certificate in Aged Care NursingMaster of Nursing (Aged Care)Specialisation in Dementia Care courses or certificationsPalliative Care course or certificationMental Health Nursing CertificationCertificate IV in Training & AssessmentClinical RolesCollection and collation of clinical data and where necessary, undertake assessments of patients, including physical examinations, health history, and medication reviewsIn collaboration with clinical aged care staff, identify changes in health conditions and take appropriate action, including contacting general practitioners and other healthcare professionals as neededFacilitate the development, implementation and review of individualised care plans based on residents' health needs and preferencesAdminister medications, treatments, and other medical procedures as prescribed within their scope of practice and where requiredProvide health information and education to older people and/or their carers in RACH regarding chronic health conditionsUnder the guidance and direction of the treating GP, develop, implement and review care plans for patients with chronic conditions, such as diabetes and heart diseaseLiaise with allied health specialists, clinics, hospitals and others to enable and supportProvide preventive health services, including vaccinations, screenings, and health educationMaintain accurate and comprehensive patient records, including assessments, treatment plans, and progress notesCare CoordinationSupport older adults and their families to identify their care requirementsAdvocate for, and promote, the rights and needs of older adults with aged care providers and other servicesMentor other staff within the General Practice clinical and aged care services to promote the health and wellbeing needs of older peopleProvide information about the local RACHs and different models of care offeredProvide information about other local services to support older peopleProvide orientation, education and information to General Practitioners, where needed, on:The aged care system including the Aged Care Assessment Service (ACAS)The local aged care service, palliative care and service eco-systemAdvanced Aged Care and end-of-life planning practices and legislative requirementsAged care funding models and AN-ACCParticipate in community outreach programs to promote aged care services and support for elderly individualsProvide education and health promotion to non-clinical aged care staff about common health conditions, medications, and self-management strategiesFacilitate and coordinate care with aged care providers including RACHs, IHC and Retirement Living, to ensure continuity of careSupport older adults to initiate an Aged Care Assessment Service (ACAS) as requiredProvide information about the local In-Home-Care providers and other services availableApplication ProcessYour application will include the following questions:
