The Rapid Response service is looking for Rapid Response Assistants to work in a multi-disciplinary team of professionals delivering a 24-hour response service to avoid hospital admission and facilitate hospital discharges. You will carry out person-centred care offering practical support and advice.
This is a developing service with great career prospects and training on offer. You must hold a valid driving license and have access to a car.
The service is open Monday to Sunday from 8am to 10pm, 365 days including Bank Holidays.

Experience in the care sector is required.
