Fixed term full time About the Role The Project Lead, in liaison with key stakeholders across the Grampians region health care services, Primary Health Network and community health services develop models of care and a workforce profile that will:
Expand the capacity of current community-based programs and health care so that where safe and appropriate, services will be delivered outside of the hospital setting.Utilise technology to build virtual ward capacity.Utilise telehealth projects across the Grampians.Streamline pathways to optimise discharge and referral into the local health services or directly into the patient's home.Key Responsibilities Investigate the regional workforce requirements (Medical, Nursing, Allied Health, Pharmacy etc) for a community-bed based model of care, with an immediate focus on acute, sub-acute and NWAU funded models.In close consultation with leaders, colleagues and key stakeholders develop pathways for the clinical deterioration of specific patient groups identified by the regional governance group.Develop clinical escalation pathways to support staff working in health care services.Develop workforce profiles to inform required people and other resources for the project/program being redesigned.Skills & Experience Demonstrated experience in development, innovation, project work and change initiatives in a clinical setting.Experience in public health industry with a high level of understanding of the applicable DHHS policies, including the Policy and Funding Guidelines.Experience in developing strategic change projects and development opportunities to a successful outcome as an expert consultant.Current knowledge of contemporary health system, professional and practice issues.Interested?
