The position: In this role, you will pay a key role in leading the work to enable the expansion of our at Home services. This will include developing Models of Care for Geriatric Evaluation Management at Home, Rehab at Home and Palliative Care at Home, along with expansion of current acute care options leading into a full Business Case. Working alongside the Better at Home team as well as the Allied Health/Rehabilitation teams, the Better at Home Expansion Lead will link into key bodies of work with stakeholders such as Public Health Services, the Department of Health and the Regional Better at Home Project Lead, as well as current acute services. Key criteria required: Tertiary qualifications in a relevant discipline, and/or demonstrated relevant health experience. Minimum 2 year's clinical or other experience working in a health service.
