We are now ready to expand our team, and we are excited about our plans to grow. We are looking for a dedicated and compassionate individual to join our team as a Placement Liaison Officer.
About the Role: This position will initially be filled as Permanent Part-time Role at 25 hours per week commencing in January 2025. The successful candidate will work closely with the Director and Retirement and Aged Care Specialist. The Placement Liaison Officer is the conduit between our Retirement and Aged Care Specialist and our clients. The Officer will be responsible for supporting the seamless transition of our clients into retirement living or aged care.
Key Responsibilities: Placement Coordination: Research and identify retirement living and aged care options within the desired geographic location of clients. Work within clients' financial boundaries to explore suitable placement options without compromising quality of care. Liaise with various retirement living and aged care homes to gather information on available services, amenities, and pricing. Communicate with facility representatives to confirm their ability to accommodate the specific care needs of clients. Facility Visits:Arrange facility tours with the Client or Family – providing transportation with door-to-door service.Communicate and liaise with retirement living and aged care homes to ensure the facilities can meet the needs of our clientDocumentation and Reporting:Organise and update client information to ensure all records are accurate and accessible. Adhere to data protection and internal policies to safeguard client information. Accurately complete and submit necessary client applications, ensuring all documentation is thorough and meets required standards.Client Support and Advocacy About You: You will play a vital part in assisting clients and their families in finding suitable retirement living and aged care placements, tailored to their specific needs and preferences. You are someone who is passionate about making a difference in our community, through nurturing long-term relationships with clients, strategic alliances and referral partners.
Utilising your empathetic nature, excellent communication and organisational skills, you will facilitate a smooth placement process, ensuring that each client receive the highest level of care and support. You need to think outside the box, be resourceful, a team player and yet work autonomously when needed. Qualifications and Skills: Strong understanding of clinical terminology, healthcare practices and aged care. Excellent communication and interpersonal skills, with the ability to build rapport with diverse clientele. Highly organised with exceptional attention to detail. Ability to prioritise tasks and manage multiple placements concurrently. Proficient in Microsoft Office Suite and CRM systems.Valid driver's license and access to a reliable vehicle for travel to facilities. Empathetic, patient and understanding approach to client care.
