Employees classified at this level are clinical leads within their area and stream of practice and have a significant degree of autonomy and decision making.The Physiotherapist will work collaboratively within CALHN IC Geriatric Services including Geriatrics in the Home, Multidisciplinary Community Geriatric Services and RACF In reach.
The role will be required to work in the community, home setting which may include the persons usual, or temporary residence including RACF.This position will form a part of the CALHN IC Geriatric Service and is part of the multidisciplinary team providing culturally appropriate comprehensive clinical intervention, assessment and care coordination, with a focus hospital avoidance, supported discharge, and hospital substitution.
The physiotherapist may be required to work within Integrated Care, as required in line with clinical need.The role will have a specific focus on older clients with complex health needs who are experiencing gaps in health responses and will provide intervention with a restorative, resilience building focus.Employees working within this level develop from novice practitioners to a proficient level of professional practice.
They consolidate knowledge and skills and develop in capability through continuous professional development and experience.
