As the Physiotherapist, you will participate as a member of the GEM at Home team based at the Repat Health Precinct, which provides care to older people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission.
About the role: In this role as a Physiotherapist with SALHN GEM at Home, you will be instrumental in delivering exceptional care to patients in their homes.
Utilising your expertise in physiotherapy, you will contribute to provision of geriatrician-led, multidisciplinary assessment, restorative interventions, and sustainable care planning in collaboration with the older person, their family, carer and existing community services involved in their care.
Duties for the role will include: Collaborate with the Geriatrician led multidisciplinary team to assess and develop restorative interventions and effective care plans tailored to the needs of older adults, their families, and caregivers.Assessment, treatment plans and implementation focusing on assisting individual consumers to optimise management of their medical condition and functional impacts as well as establishing community-based support networks and/or suitable supported accommodation.Provide service/peer leadership in line with the Division of RAP About you: A degree or other qualification which entitles registration as a Physiotherapist with the Physiotherapy Board of AustraliaAbility to assess and provide appropriate Physio-therapeutic management to the patients of the GEM at Home service with increasing levels of complexity Evaluate issues related to Physiotherapy care in a manner which can be applied to quality activities, service evaluation, professional education or research with a commitment to ongoing professional developmentHold a current South Australian driver's license and willing to drive a government plated vehicle in the community.
