The South East Grey Community Health Centre (SEGCHC) is a primary care facility located in Grey County with five sites located in Dundalk, Flesherton, Markdale and Chatsworth. The SEGCHC is dedicated to meeting the broader needs of the communities in which we serve through the development and implementation of primary care, health promotion and community development best practices.

Description: We are currently seeking a part-time to full-time permanent Physician (30.0 hours - 37.5 hours per week), who as a valuable member of the Community Health Centre team, will provide primary care to clients and their families within a care model which emphasizes health promotion/education and empowerment.
The South East Grey CHC received the highest patient satisfaction rating of all of the South West LHIN CHCs - a 96% patient approval rating. Additionally, we are the most efficient CHC in Ontario on a cost per patient served basis and cost per patient visit. We currently have four physicians and ten nurse practitioners serving more than 8,000 patients on a full and part time basis.
The primary care practice offers in-house physiotherapy, social work and dietitian (nutrition counselling) services. Additionally, we offer health promotion and community development services through more than forty community-based programs.
The South East Grey CHC has several specialists working from our site in Markdale including an internist. We work very closely with our hospital partners to ensure that patient and community needs are met.

There are additional opportunities to work with our physician colleagues at Grey Bruce Health Services in Markdale in the ER and/or in-patient if so desired.
Qualifications: Medical degree and family residency from a recognized university.Current CPSO license and CMPA registration.Experience in a combination of hospital and community settings and/or public health.Demonstrated commitment to and knowledge of community-based health care.Excellent communication and interpersonal skills.Ability to work independently and collaboratively in a multidisciplinary setting.Ability to work effectively with marginalized and under serviced populations.Salary: To be negotiated
