Key Responsibilities: Conduct thorough assessments to establish patient needs and therapy goalsCreate tailored treatment plans based on comprehensive evaluationsAdminister interventions and therapies to enhance patient mobility and independenceMonitor patient progress and adjust treatment plans as necessaryEducate patients and their families on exercises and health strategiesCollaborate with healthcare professionals to provide holistic careComply with all regulatory requirements and best practices in home healthcareFull-time schedule with local travel requiredSchedule: Weekdays: Monday to Friday, 8 AM to 5 PMWeekend work: One weekend per monthNo on-call responsibilitiesQualifications: Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited institutionCurrent state licensure to practice as a Physical TherapistMinimum of 1 year of relevant experience in home health care is preferredProficiency in HCHB and OASIS documentation is highly desirableStrong clinical reasoning and assessment skillsExcellent communication and interpersonal skillsAbility to work collaboratively as part of a multidisciplinary teamCommitment to providing exceptional patient careBenefits: 401(k) programDental, vision, medical, & life insurance optionsGenerous PTO programLeadership & Development opportunitiesEducation assistance #J-18808-Ljbffr
