We pride ourselves on providing trauma-informed, person-centred, and consumer-driven care that allows people to take control of their lives and bring about meaningful change.We are seeking a Family Violence Perpetrator Case Manager to join our team!About the RoleThis position provides short-term individual case management services to people using family violence (those who are perpetrating family violence).The purpose of the case management program is to:Keep people using violence in view of services and relevant authorities to contribute to a 'safe at home' approach to promote the safety of victim survivors (adults and children).Identify information that may be proactively or responsively shared on request under the information sharing scheme to contribute to risk assessment and management for victim safety.Offer clients a timely response.Actively engage clients to identify key changes necessary to enable perpetrators to take responsibility and enable the victim survivors to live safely, including the ongoing use of risk assessment and risk management.Provide a more tailored service response through the coordination of specialist services, including mental health, alcohol and other drug, and housing services.Assist in engagement with programs that aim to stop family violence, including but not limited to Men's Behaviour Change programs.Actively involve the client in planning and decision-making to encourage engagement with other social and universal services.The service areas for this position include the Local Government Areas of Greater Shepparton, Strathbogie, and Moira.This position reports to the Manager Family Violence and works within the Family Violence Team. It is also expected that this position works within the larger Primary Care Connect team and networks with external organisations.Key Selection CriteriaThe following criteria must be met and addressed in applications for consideration for this position:Bachelor in Social Work or equivalent, or relevant qualifications and a willingness to work towards an equivalent qualification as set out in Mandatory Minimum Qualifications - Qualifications Mapping.Graduate Certificate in Client Assessment and Case Management or a willingness to undertake.Experience in working with perpetrators of family violence to increase their accountability for their use of violence and in promoting the safety of victim survivors, including children.Experience using the Multi-Agency Risk Assessment Management Framework and information sharing legislations.Ability to undertake client-centred assessments and develop a support plan in partnership with the client that reflects their goals.An understanding of the social and gendered context of family violence and the impact of violence on victim survivors.Professional and personal alignment with Primary Care Connect core values.Benefits and PerksPrimary Care Connect provides a supportive workplace with staff who are committed and passionate about the community that we work in.
