Join our compassionate team in Keswick, South Australia, as a full-time Clinical Practice Consultant (CPC) in Palliative Care. This permanent role includes travel to various sites as needed.
Role Overview: Clinical Practice Consultant will play a crucial role in providing clinical leadership, guiding nurses in the development, execution, and assessment of evidence-based, patient-centered care models. You will oversee training, manage clinical practices, and conduct individual competency assessments for staff assisting clients with complex health challenges in the community. Your contributions will be vital in enhancing clinical care, service efficiency, and client outcomes.

About You: You are committed to excellence and passionate about positively impacting others' lives. To be considered for this position, you should have:
Post Graduate in Palliative Care Nursing + 2 years Post Graduate Experience
Specialized skills and knowledge relevant to community nursing and the demands of this role.
Advanced expertise in planning and executing nursing care relevant to your responsibilities.
In-depth understanding of Primary Health Care Principles, Chronic Disease Management, and Self-Management strategies.
