We are seeking experienced and passionate Paediatric Road Nurses to join our community team. We are seeking dynamic clinicians who can work on road within the South Australia Community Care (SACC) program.

Using your clinical skills, you will ensure a comprehensive client centred management plan is achieved for our consumers. You will support and provide care and services to SACC consumers for HSS within the community setting delivering care to patients in their home.

• Customers initial assessment, care planning and goal setting
• Undertaking home visits for clients within the SACC program
• Monitoring patient progress and updating care plans as appropriate
• Liaison with healthcare professionals and specialists
• Discharge planning including referrals to long term services
• Provision of telephone support, education, and advice

These positions will be offered on a casual or perm basis working across a 7-day roster between the hours of 7am and 8pm. We have work available in all areas of Adelaide.

What will set you apart from the rest? You will be client and goal focused who is driven to achieve positive customer outcomes together with:
• Registration as a Registered Nurse with AHPRA (unrestricted)
• Recent paediatrics experience in an acute or community setting
• Strong clinical reasoning skills
• Knowledge and experience in:
o Wound care/ Negative pressure
o IV Antibiotics
o PICC line management
o Respiratory assessments
• Ability to work in a fast-paced environment
• Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
