Physical Rehabilitation Care Coordinator - (FT) The Care Coordinator in the CHP Physical Therapy Department is primarily responsible for facilitating smooth patient flow and supporting the operational needs of the outpatient clinic. Key duties include getting initial paperwork and forms signed, taking phone calls, explaining benefits, scheduling patient appointments, collecting payments for deductibles and copays, and making reminder calls to patients to confirm appointments. The Care Coordinator also assists with bringing patients into the clinic, setting them up for their sessions, and guiding them through initial exercises as directed by the therapist. Handles sensitive information with confidentiality and maintains the integrity of patient financial and medical records. The Care Coordinator will cross-train with the office coordinator and act as back-up, as needed.
Job Specific:
Serve as the receptionist for the department, answering, screening, and directing phone calls, emails, mail, and faxes, and welcoming and assisting visitors. Process payments, record patient transactions for deductibles and copays, and ensure revenue entries are accurate prior to management review. Obtain referrals, explain insurance benefits, and schedule appointments for clinical patients. Schedule patient appointments, confirm appointment details, and communicate necessary information, including clinical test needs and physician reports, to ensure continuity of care. Retrieve necessary patient information, data, and formulate patient medical records. Secure necessary clinical tests, results, reports, and signatures from physicians. Establish and maintain timely communications with physicians and physician's office staff. File physician correspondences and other patient medical record information in a timely manner. Understand patients' needs and personalities to appropriately match patients with the most suitable therapist, ensuring a high level of patient satisfaction. Compile and record medical charts, reports, and correspondence, using available office equipment and methodologies. Facilitate cross-functional communication and coordination as it relates to policies and procedural requirements of the department. May provide specific patient treatment only as directed by the staff physical therapist and physical therapist assistant. Hospital:

Required: High school diploma or equivalent.
EXPERIENCE
Preferred: Experience in a fast-paced secretarial or clerical office setting; medical secretary and money handling experience.
SPECIAL SKILLS AND ABILITIES
Required: Excellent verbal and written communication skills; excellent interpersonal and customer service skills; ability to multitask and adapt to interruptions; proficient in Microsoft Office Suite to include Outlook, Word, Excel, PowerPoint, and Teams; excellent organization skills and attention to detail; ability to work independently.
Preferred: Basic understanding of clerical procedures and systems such as recordkeeping and filing.
