This is an exciting opportunity to be part of a dynamic multidisciplinary team, helping to shape and implement an innovative model of care that will set new standards in patient outcomes.Aligned with CALHN's strategic vision for delivering world-class healthcare, Hampstead Centre is launching a new program, "CO-ACT" (Care of the Older Person and Community Transition), focused on providing exceptional care for older adults transitioning from acute to ongoing care.
As a member of our CO-ACT team, you'll work in a supportive, family-like environment where collaboration is key, and your input on improving systems and practices is highly valued.About the RoleAs an Occupational Therapist with the Care of the Older Person and Community Transition (CO-ACT) service at Hampstead Rehabilitation Centre, you will play a vital role in supporting patients through specialised assessment and intervention.
Working independently, you'll apply advanced skills to manage complex, high-acuity caseloads, ensuring each patient receives high-quality, personalised care aimed at enhancing their independence and overall well-being.In this dynamic role, you'll collaborate closely with a multidisciplinary team, contributing to effective and efficient patient care.
You'll also have opportunities to engage in quality improvement and research initiatives, helping to drive innovation and enhance service delivery standards.
This role is ideal for a proactive Occupational Therapist looking to grow professionally while making a meaningful impact on the lives of older adults.About YouYou'll be a great fit for this role if you have: Strong communication skills to engage effectively with patients, carers, and team members.Proven clinical expertise and judgement in managing complex cases across acute, sub-acute, and community settings.Demonstrated adaptability, organisational skills, and initiative in high-paced environments.Experience supervising students and staff and a commitment to ongoing professional development and best practice.
