The Integrated Care AHP2 Occupational Therapist will promote a 'home first' approach and provide comprehensive assessment, short-term intervention and care coordination to complex consumers with a focus on:
Hospital avoidance, supported discharge, including transition to community-based services and hospital substitution. Occupational therapy in-home reviews and assessments form an important 'home in-reach' arm of our model, particularly for the CALHN Bright service model and Hospital in the Home. Key services provided include:
Home Safety Assessment Functional and/or cognitive assessment in the home Patient education - Falls prevention & energy conservation education. Discharge equipment setup and review Basic home modification recommendations Pressure area equipment set-up and education Onward referrals for ongoing supports (e.g. My Aged Care (MAC) for community ACAT/RAS, etc.) The Occupational Therapist requires well-developed clinical skills and knowledge to provide culturally appropriate care to consumers with complex health needs and who may be experiencing gaps in health care. The Occupational Therapist will have a thorough knowledge of referral pathways and available community services, extensive experience working within a multidisciplinary team, and highly developed communication skills to enable effective liaison and collaboration between the consumer, Public Sector, and Non-Government.
About you
Required to work across CALHN sites and within Integrated Care, as required and in line with clinical need. A degree or other qualification which entitles registration as an Occupational Therapist with the National Board OT Board of Australia through the Australian Health Practitioner Regulation Agency (AHPRA). Highly developed ability to communicate effectively (both verbally and in writing) with clients, carers, staff, and relevant others. Clinical experience working in Occupational Therapy within an acute tertiary hospital, subacute rehabilitation hospital, mental health, or primary care, community, outreach, or ambulatory rehabilitation setting. Knowledge of current Occupational Therapy methods in an acute hospital, sub-acute, mental health, or community, outreach, or ambulatory based setting. About CALHN
