Job DescriptionGreatness starts here Working as part of the Adult Occupational Therapy team, and reporting to the Occupational Therapy Team Leader, you will be responsible for:
Providing client centered care with a high standard of clinical services and expertise to adult clients with culturally and linguistically diverse backgrounds. As part of this role your day-to-day responsibilities will include:
Working as an active member of the OT team, providing holistic client centred OT care to members primarily of the Hume and Whittlesea communities.Applying evidence-based clinical skills and knowledge in the management of a diverse caseload with a focus on prevention, early intervention, restorative and strength-based interventions to facilitate the maintenance and/or improvement of activities of daily living.Working in a multidisciplinary team to develop and implement client centred care plans.Be involved in quality assurance and service development projects to enhance innovative and efficient practices.Supervising Grade 1 staff members, OT students and Allied Health Assistants where appropriate.Liaising with external services/suppliers, 3rd parties and partners to ensure a quality service is provided to our communities.Actively engage in team meetings and case discussions both within the OT team and the Health and Wellbeing team.Desired Skills and ExperienceBring your great, everyday! To be successful in this role you will bring demonstrated experience working within a multidisciplinary team managing complex clinical presentations relating to the restoration or maintenance of functional capacity in clients with chronic medical conditions and physical and/or psychosocial disabilities.
You will possess a range of skills and requirements, including:
Qualification in Occupational Therapy with registration as a Occupational Therapist with AHPRA.Sound theoretical knowledge base and the commitment to continuing Professional Development to ensure best practice/evidence-based interventions.Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with your clients.A minimum of 3 years working as an Occupational Therapist, preferably in a community setting working with CHSP, HACC, HCP and NDIS funding. Applicants with less experience will be considered.Enjoy working within a multidisciplinary and interdisciplinary team environment.You will have proven ability to deliver a service in line with best clinical practice.Have a clear understanding of community health and values that reflect working in community health.Experience of running group OT intervention.Excellent written and verbal communication skills, including detailed report writing.Autonomous, with good caseload and time management skills.Experience working with people with chronic health conditions, physical and/or psychosocial disability.The fine print Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.Full driver license and travel between sites required.To view a copy of the Position Description please click here.Applications close: We will be reviewing applications as they come through and we may take the ad down earlier if a suitable candidate is identified.
